I was never asked any Medical information or Physicians names. I think that it was just wanting to sell me insurance.

It didn't ask for Medical information even after I signed in to Yahoo. It told me my form was sent. So how is it going to look like the card on the picture?

All it would do for me was tell me what info I gave them. It didn't ask for RX's, Doctors or anything other then my name and address

Why must I sign into Yahoo in order to go to the next step? And why isn't the first screen asking for the RX information? I'm afraid that they just want to get your information so they can try to sell you insurance. Anyone else have these issues?

I had no issues with this one. Went right to the form to have the card mailed. Was very easy actually.
