| I couldnt get my birthdate to work-I followed the format the form required for my birthday and it kept telling me to check it. I put it in 3 different times in the correct format and it would not work-format is mm/dd/yyyy. |

| my information went right thru. there was no birhdate to put in except for the year of birth. |
